In fact, there are so many variations of OCD that it is impossible to list them all, but here are some of the most common types of OCD that we treat: Fears of getting dirty or catching a disease: Often this leads to fear of coming into contact with bodily fluids, germs, or chemicals. For some, it just means being afraid of feeling dirty. Others may worry about being responsible for getting other people contaminated. In any case, if you have contamination fears, you probably avoid touching many things, especially in public. This may keep you from going to certain places or participating in social outings. You probably wash or sanitize your hands frequently in order to feel clean. Fears of harming others: Some people with OCD are tortured by thoughts about other people being harmed or even dying. It might even feel like you can cause this to happen just by thinking about it. You probably feel guilty and wonder why you think such terrible thoughts. Religious obsessions: If you are a spiritual or religious person, you may have unwanted thoughts that could be blasphemous in your religion or offensive towards God. We have offices in multiple cities and we are seeking a new therapist for our Boise office. We offer all the perks of private practice (good pay, flexibility in setting your schedule, lots of vacation time, generally a low-risk clientele to work with) but without any of the pain points of private practice (having to do marketing to get clients, administrative work, startup costs and overhead, managing a business). You also get to be part of a great team of people to work with and have consultation and support readily available. We take care of all of the work of marketing and getting clients. We have a dedicated administrative staff that handles scheduling and billing (for all of our offices), so administrative work for our therapists is kept to a minimum. Other than training/consultation and minimal paperwork requirements, your time will be spent doing the actual clinical work. Your client load will be almost entirely clients with anxiety disorders and OCD. A small percentage of your caseload will also be working with PTSD, trichotillomania, and skin picking disorder. All treatment is one-on-one individual therapy. Most of your clients will be adults but we would ideally like someone who is open to seeing adolescent clients as well. Someone open to seeing younger children would also be a major plus. We are most interested in finding someone who already has specialized training and experience treating anxiety and OCD using exposure therapy, but we are open to applicants with less experience with this specialty. We also have a strong emphasis on Acceptance and Commitment Therapy, so if you have experience with ACT, that is helpful as well.  Depending on level of prior experience with anxiety/OCD, exposure therapy, and ACT; there will be a training/case consultation component for the first 6 months to a year to train you up on doing this type of work. We would ideally like to find a licensed psychologist but are open to Master's-level therapists as well.  At minimum, a master's or doctoral degree in clinical or counseling psychology or social work is required. Starting compensation would also be based on experience level. Our therapists' compensation is based on a percentage of the fees that they collect from clients. If you are a psychologist who already has experience and training in treating anxiety and OCD, you would earn 60% of the fees that you collect. We will go over the full math of how this works out during your interview, but with a full caseload, that comes out to $100,000 per year. Those with master's degrees and/or less experience and training in anxiety and OCD would start at 50 to 55% of fees collected, depending on degree and level of prior experience, and eventually go up to 60% after training is complete. These behaviors may seem odd and confusing, but we see them in the children and teens we treat all the time. Fear of getting sick or being dirty (fear of “contamination”):  Many children with OCD worry about coming into contact with germs, bacteria, or other people’s bodily fluids. Others worry more generally about just being dirty.Common behaviors parents see in children with this fear: Repetitive hand-washing. Refusing to touch certain things. Reluctance or refusal to go to school. Avoidance of “dirty” people or places. Getting angry when family members touch their possessions. Intrusive thoughts about terrible things happening:  Children with this type of OCD have intrusive thoughts about dreaded events like the death of a family member or being kidnapped. They may fear that they can cause these things to happen just by thinking about them, unless they prevent it by doing rituals.Common behaviors parents see in children with this fear: Repeating simple, routine actions (e.g. opening and closing doors, standing up and sitting down). Doing things repeatedly a certain “good” number of times.  Praying that the thought does not come true. Repeating things until they feel “just right”. Asking for reassurance (e.g. “Are we going to be ok?”) over and over. Taking a very long time to do simple tasks like getting dressed, taking a shower, or getting ready for bed. Taking a very long time to do homework. Checking doors to make sure they are locked. Checking light switches to make sure they are off. Checking to make sure the stove is off. Checking that things are not dirty or contaminated. Praying to “undo” bad thoughts. Repeating simple actions (e.g. opening and closing doors) until they can do them without having a bad thought. Reluctance to attend religious services out of fear that it will trigger bad thoughts. I am not afraid of something simple and physically concrete like dogs, can Exposure Therapy still work for me? Yes, absolutely. Although we use that fear as an example on this page about Exposure Therapy, very few clients that we see are coming in for such a simple phobia. Typically, if you are not sure of specific situations and triggers for your anxiety, the trigger is really just uncertainty of any kind. It could be uncertainty about your future, whether you will be able to keep a job or relationship, why you are having certain thoughts, why you feel anxious, or really anything. Even though uncertainty is not a physical thing we can put in front of you to do exposure therapy with like a dog, the idea of exposure works exactly the same way. We simply need to get you exposed to feeling uncertain without doing anything to resolve it. In other words, it’s exposure to particular thoughts about uncertainty rather than a physical object. Remember that short-term avoidance of anxiety leads to long-term maintenance of anxiety. The way this works with uncertainty is that when we feel uncertain about something, the way our brains tell us to “run away” from it is to analyze it so that we can feel reassured, resolved, and certain about it. This leads to excessive worry, rumination, and overanalysis because you are trying to get a for-certain answer to something that cannot be answered for certain. The solution is to learn the skill of cutting off the process of mentally analyzing worry questions and instead moving on with your day while purposely leaving these questions unresolved, unanswered, and uncertain. If you do this, you will be exposed to uncertainty and will get used to it the same way someone with a dog phobia would get used to dogs. Believe it or not, you can eventually reach the point where uncertainty is no longer a trigger for anxiety for you. Basically, instead of exposing yourself to a physical object like a dog, you would be exposing yourself to thoughts about uncertainty, but without the avoidance you usually do via mentally analyzing. This would give your brain a chance to re-learn that such thoughts are not actually dangerous and do not require any action, which is how the exposure works and the anxiety gets better (see below to learn why this hasn’t happened for you already). That is the very quick, short explanation of this, obviously we will go into much more detail about it when we meet with you. But the short answer to this common question is that yes, exposure will still work for fears like this and it is actually more common for folks to be coming to us for help for something like this than for a more concrete, simple phobia.  We know what to do with it. I don’t even know what triggers my anxiety, it seems like it just happens all the time! How can we do exposure if we don’t know what the trigger is? Anxiety triggers can often be difficult to identify, which is why it can seem like the anxiety comes out of nowhere. But there is always a trigger. Anxiety does not just happen randomly even though it may seem that way sometimes. We will use our extensive experience with anxiety clients to help you identify subtle triggers, which we will then use for exposure exercises. I get exposed to the things I’m afraid of all the time. It hasn’t worked so far. Why would Exposure Therapy work for me? This is a good question that we get all the time. You are correct, you’ve already had lots of exposure to the things that make you anxious, otherwise you probably wouldn’t be seeking help. However, the problem is that when you get exposed, there is something you are doing to avoid the trigger or the anxiety itself. And any avoidance prevents exposure from working. Avoidance behaviors can be subtle and you may not even realize what you are doing to avoid the anxiety. It may even seem like you are not avoiding at all. But there is no such thing as an anxiety problem without some form of avoidance. Even doing anything to partially avoid or resist the anxiety or the anxiety triggers can prevent exposure from working. Most people are not aware of how they are avoiding, which is where we can help. We will help you recognize and eliminate avoidance behaviors you may not be aware of that are maintaining your anxiety. Then your exposure will work and your anxiety will decrease. Therapy has not worked for me before, why would this be different? Firstly, don’t worry, the vast majority of our clients are coming to us having had other attempts at therapy fail for them before. We are often a last resort for people. Most of our clients have been to what is typically referred to as “talk therapy”, where the therapist listens to the client and perhaps offers interpretations of what the client is feeling or the origins of their issues, but does not offer any concrete advice about what they should do about it. Most of our clients who have been through this type of therapy will say that while their therapist may have been very nice, it did not ultimately do much for their anxiety. While talk therapy has its place for some issues, it is often not enough for anxiety. Our clients will also often say that if their therapist did give them specific advice, it was typically to do some relaxation exercises, which also usually were not effective. Even worse than these attempts at these therapies not being effective is the fact that they often leave clients feeling that they must have a more serious, untreatable issue that cannot get better or that they themselves were not doing the therapy right somehow.  If you have been through a similar experience, rest assured that the issue was likely not with you but with the fact that you weren’t receiving the kind of specialized help you need. Exposure Therapy is different because the therapist takes a much more active role. We will give you very clear, straightforward advice on exactly what to do to fix your anxiety issue and reach your goals. Although we use that fear as an example on this page about Exposure Therapy, very few clients that we see are coming in for such a simple phobia. Typically, if you are not sure of specific situations and triggers for your anxiety, the trigger is really just uncertainty of any kind. It could be uncertainty about your future, whether you will be able to keep a job or relationship, why you are having certain thoughts, why you feel anxious, or really anything. Even though uncertainty is not a physical thing we can put in front of you to do exposure therapy with like a dog, the idea of exposure works exactly the same way. We simply need to get you exposed to feeling uncertain without doing anything to resolve it. In other words, it’s exposure to particular thoughts about uncertainty rather than a physical object. Remember that short-term avoidance of anxiety leads to long-term maintenance of anxiety. The way this works with uncertainty is that when we feel uncertain about something, the way our brains tell us to “run away” from it is to analyze it so that we can feel reassured, resolved, and certain about it. This leads to excessive worry, rumination, and overanalysis because you are trying to get a for-certain answer to something that cannot be answered for certain. The solution is to learn the skill of cutting off the process of mentally analyzing worry questions and instead moving on with your day while purposely leaving these questions unresolved, unanswered, and uncertain. If you do this, you will be exposed to uncertainty and will get used to it the same way someone with a dog phobia would get used to dogs. Believe it or not, you can eventually reach the point where uncertainty is no longer a trigger for anxiety for you. Basically, instead of exposing yourself to a physical object like a dog, you would be exposing yourself to thoughts about uncertainty, but without the avoidance you usually do via mentally analyzing. This would give your brain a chance to re-learn that such thoughts are not actually dangerous and do not require any action, which is how the exposure works and the anxiety gets better (see below to learn why this hasn’t happened for you already). That is the very quick, short explanation of this, obviously we will go into much more detail about it when we meet with you. However, the problem is that when you get exposed, there is something you are doing to avoid the trigger or the anxiety itself. And any avoidance prevents exposure from working. Avoidance behaviors can be subtle and you may not even realize what you are doing to avoid the anxiety. It may even seem like you are not avoiding at all. But there is no such thing as an anxiety problem without some form of avoidance. Even doing anything to partially avoid or resist the anxiety or the anxiety triggers can prevent exposure from working. Most people are not aware of how they are avoiding, which is where we can help. We will help you recognize and eliminate avoidance behaviors you may not be aware of that are maintaining your anxiety. Trichotillomania and Excoriation Disorder fall under a category of behaviors called Body-Focused Repetitive Behaviors. They are characterized by repetitive, impulsive behaviors that are hard to resist in which pulling, picking, scratching, or scraping of the skin, nails, or hair result in damage to the body or noticeable hair loss. Common Symptoms Recurrent pulling of one’s hair, sometimes leading to hair loss Recurrent skin picking, sometimes resulting in skin lesions Repeated attempts to decrease or stop hair pulling or skin picking Skin picking or hair pulling causes significant distress or impairment in social, occupational, or other important areas of one’s life Trichotillomania and Excoriation Disorder Treatment Both of these disorders are best treated by unique forms of Coginitive-Behavioral Therapy (CBT), which help reduce the behavior itself as well as its social and emotional impact. The software we use for online therapy sessions will only work in Google Chrome, Firefox, or Safari, so make sure you are using one of these web browsers.   It will not work in Internet Explorer/Microsoft Edge.  Here is a link for downloading the Google Chrome browser if you need it: Google Chrome 2. If possible, hook your computer up to the Internet directly via an ethernet cable rather than using Wi-Fi. While Wi-Fi can work, it is less reliable and sometimes slower. If you are going to try to use Wi-Fi, make sure you are close to the router and that no other devices on the network are using a lot of bandwidth. 3. We recommend using a computer/laptop rather than your phone or tablet. While phones and tablets do often work just fine, we have experienced problems with using them in some cases. Please understand that you are taking a risk if using your phone or tablet; the call quality may not be sufficient to have a session. 4. Test your device's capability to have an online therapy session beforehand. Security Of Your Sessions There are 2 primary forms of protection to maintain the security of information online. Both of these methods are used to protect your confidentiality during your sessions with us online. Encryption is scrambling information that is transmitted online to make it unreadable and unusable to anyone trying to intercept it.  Our online therapy platform, Doxy.me, uses 128-bit encryption, which is considered very secure. It is up to your discretion whether you would like to use additional encryption methods on your computer as well. A firewall is a hardware or software filter meant to ensure that only authorized information is sent in or out of a computer or network. We use a firewall on our computers. A firewall is an extra step of security that you can take if you wish. We use a firewall on our end, and it is up to you if you wish to use one on your end. Windows and iOS both come with firewalls, though you may need to make sure they are active.  Additional firewalls can be found in home computer security software products like Webroot or McAfee. Please understand that although we make these efforts to ensure that your online therapy session transmissions will not be intercepted, there is no such thing as a 100% foolproof online security plan. We cannot guarantee the security of any information exchanged online. Finally, it is your responsibility to ensure confidentiality in your physical location. Make sure you are in a quiet area, preferably at home, where no one else will be able to overhear your session. #### Our Approach [vc_row][vc_column width="2/3"][vc_column_text] Exposure Therapy is the most EFFECTIVE treatment for anxiety and OCD. How Anxiety Works Right now, your brain looks at the situations, thoughts, objects, and places that trigger your anxiety and decides that they are dangerous. It then uses anxiety as a warning signal to alert you to the danger so you can escape from the trigger in some way in order to reduce the anxiety. This is called avoidance, and it is the cause of any anxiety problem. Avoidance of anxiety and the things that trigger your anxiety keeps the anxiety going in the long run because it prevents your brain from learning that your anxiety triggers are not actually dangerous. When you avoid the things that make you anxious in the short-term, your anxiety gets worse in the long-term. We Help Re-train Your Brain so That it Reduces or Eliminates Anxiety In order to fix the problem, you must re-train your brain to understand that your anxiety triggers are not actually dangerous. This can only be done through direct experience…after all, you’ve probably tried talking yourself out of anxiety before or tried to “think rationally” already, right? It just makes you feel crazy because you think “I KNOW this is irrational! I KNOW I shouldn’t be so anxious about this, but I can’t help it!” Well unfortunately, you can’t talk your brain out of an anxiety problem. Your brain needs to be convinced by directly experiencing that the anxiety trigger is not actually dangerous. This is what we do in Exposure Therapy. This may sound scary, but rest assured that we will do it at your pace and guide you every step of the way. We will show you how to do each exposure exercise and, step-by-step, exactly what to do to make it work. In most cases, we can do it with you during our sessions, even if it means working outside our counseling office to help you overcome something that is causing you anxiety. A Simple Example of How Exposure Works: Pete’s Fear of Dogs Note: This is just a simplified example to demonstrate the idea. But Pete’s children desperately want to get a dog. He wants to make his children happy but also does not want to live in constant fear inside his own home. Here is what is happening: Every time Pete is faced with a dog, he becomes anxious. This is because his brain evaluates the dog as dangerous, and uses anxiety to warn him that he is in danger.  In order to relieve this anxiety, he runs away from the dog. This works in the immediate short-term: in that moment, he is no longer anxious because he gets away from the dog (his anxiety trigger).  However, this strategy does not work for him in the long-term.  The next time he is confronted with a dog, he will be just as anxious or even more anxious because he avoided the previous dog rather than facing it. By avoiding the dog, he prevented his brain from learning that it is not dangerous! If he continues to avoid dogs like this, he will always be afraid of dogs and may never be willing to get a dog for his kids.  Note that Pete can try all he wants to convince himself that the dog is not dangerous by saying things to himself like “The dog isn’t going to bite me”, or “I’m going to be ok, it’s just a dog.” But even though he might already realize his fear is irrational, this will NOT change his brain’s reaction to the dog. He will still get anxious when he sees one. The only way to change his brain’s reaction to the dog is to learn through direct contact that the dog is not dangerous. Here is how Exposure Therapy would work for Pete: He sees one of our therapists, who gradually helps him spend time with dogs both during and between sessions. First he practices just being in the same room with a dog. Then he practices petting a dog. Finally, he works his way up to holding a dog. At first, each step makes him very anxious, but he stays with it.  As the counseling sessions progress he eventually gets used to being with dogs, and they no longer cause him anxiety. This is because his brain gradually learns from this experience that the dog is not dangerous.  He is now willing to get a dog for his kids and can comfortably live with this dog without feeling anxious.  This same basic process of Exposure Therapy that would work for Pete’s fear of dogs works for any other type of anxiety or OCD you may be facing, because: When you do the things that make you anxious in the short-term, your anxiety decreases in the long-term. We sincerely apologize for our inability to get you in immediately; we know that frankly, it sucks to have to go on a waitlist.  You are probably eager to get started on treatment as soon as possible and that waiting is not ideal. Since we understand the urgency of getting you in as quickly as we can, we try to take great pains to get people in to start with us as soon as we possibly can. The best way we have found to do this is to allow clients on our waitlist to schedule appointments, but with some restrictions. The upside to allowing this is that it allows clients to be able to start therapy right away even in spite of our waitlist.  The downside is that the restrictions can sometimes create some initial confusion if clients do not fully understand these restrictions. However, we find that this confusion is avoided if clients read the full explanation of these rules below, and we believe that the upside of getting clients in faster outweighs the downside of any initial confusion.  The system we use is actually quite simple, and the alternative to these rules would be that there is no way around waiting to get in with us.   So here is how it works: If you are on our waitlist, you are welcome to use our online scheduler to book any appointment available within the next 4 calendar days, but not further out than that.  For instance if today is March 15th, you could book any appointment for March 19th or earlier, but you cannot book appointments for March 20th or later. We typically go into every week with all of our appointment slots filled by established clients. But each week, a few clients will cancel here and there, opening up some available times. These canceled appointment slots are what we make available to clients on our waitlist.  We have to have a waitlist because we do not want an established client to not be able to get an appointment in a given week because we gave too many appointment slots to new clients. But we allow waitlist clients to schedule once we hit 4 days out because at that point, established clients have had a chance to book and it is unlikely any of them will need the appointment slot. This way, even though we cannot yet guarantee a weekly appointment for you, we are able to get the ball rolling much sooner and have therapy start much earlier.  We have found that clients who are proactive about this and who have relatively flexible scheduling availability are generally able to get an appointment most weeks even when they are on the waitlist. If you are on our waitlist and you book an appointment more than four days in advance, we will have to cancel the appointment. This is the scenario we really try to avoid by explaining these rules here in detail, as we know it is a frustrating experience for you if we have to cancel an appointment you booked.  We need to do this though because we really need to keep the appointments available for our established clients who are in the middle of therapy with us already.   Therefore, if you book an appointment more than 4 days in advance, we will have to cancel it in order to protect our availability for established clients.  Please know that once you are off the waitlist and are an established client on our regular caseload, your appointment slots will be protected by this policy. Sometimes, a therapist may have a waitlist only for late-day appointments (4pm start times and later), meaning they can get you onto their caseload immediately if you can regularly come in for 3pm start times and earlier. This means that if you can do appointment start times 3pm or earlier, they can get you onto their caseload right away with no scheduling restrictions. But if you require 4pm appointments or later, you will need to go on the waitlist for those appointment times and can only book appointments 4pm or later within this 4-day window.  We sometimes need to do this because demand for after-school or after-work appointments is extremely high and there simply aren't enough hours available to fit in everybody who wants those times. Getting in one time for an appointment scheduled this way while you are on the waitlist does NOT mean you can then freely schedule appointments for any date or time after that. You will still be on the waitlist and must continue to schedule within this four-day window until we let you know that your therapist has a spot available on their regular active caseload for you. The need to have your appointments scheduled in this fashion is only temporary. Once your turn comes up to get onto your therapist's regular active caseload, you will be able to book as many appointments as you want as far out in advance as you want, guaranteeing you an appointment every week.  You can always feel free to contact us to check in on how much longer we expect the wait to be to get onto the active caseload.  It just depends on when current clients finish therapy, and we can give you a range of time we expect for how long this will take before it's your turn. Thanks for taking the time to read all this and for your understanding about these restrictions.  We know it can be frustrating to want to get help right away and to not be able to.  The intent of these policies is to get your therapy started as soon as possible while still not taking appointment slots from our already-established clients.  We hope this all makes sense, but if there is anything else we can do to make any of this clearer or more user-friendly, please contact our practice owner, Dr. Michael Stein, at drstein@anxietysolutionsofdenver.com, as we are very open to suggestions for anything we can improve here. A few other helpful details to keep in mind: While you are on the waitlist and are scheduling in this way, we cannot guarantee that we will have an appointment available for you every week. It is possible that you may have an appointment one week but then have to wait a couple of weeks for another appointment that works for your schedule to become available. We know that this is not ideal, but clients tend to like having this option anyway because it is much better than not being able to get in at all while they are on the waitlist. Scheduling while you are on the waitlist is completely optional: if you feel that it does not work for you to have uncertainty about when you will be able to get in and how long it will be between appointments, then you do not need to schedule at all while you are on the waitlist.  You can simply wait for your turn to come up for a spot on our regular active caseload, and then you can schedule a recurring weekly appointment time. Scheduling appointments in this way does require you to be a bit proactive to make sure you get appointments. Appointment slots are updated immediately in the online scheduler as they are booked or canceled and are first come, first serve; so we recommend checking the online scheduler each day to see if any appointments have opened up that would work for you. On the online scheduler, you may see that the only available appointment type is "session for established client". Once you have read these instructions, you may ignore that and book it anyway, as long as it is within the next 4 days. For your first session, you are welcome to book with any therapist of your choosing (though some therapists have age restrictions). After you have already met with a therapist, that is your therapist for all of your sessions going forward, so please only book with them for all future appointments.  If you would like to know what the wait times are for each therapist, please contact us and we would be happy to give you the expected wait time for each therapist so you can make an informed decision about who to book your first session with. There is no waitlist for initial free phone consultations, you may schedule those by contacting us at any time. The therapist will learn all about you, the problems that brought you to therapy, and your history.  At the end of your first session, they will leave some time to give you some feedback about your diagnosis and what your treatment will look like. They will answer your questions as well. [/vc_column_text][/vc_accordion_tab][vc_accordion_tab title="Step 2: Psychoeducation (typically 2 sessions)"][vc_column_text]Your therapist will teach you some basic concepts that set the foundation for how to make an anxiety problem better.  You can check out our blog for a small taste of how we explain anxiety concepts to our clients. You will learn about: the brain science behind anxiety and OCD and how it is changed through the cognitive-behavioral treatment method of Exposure Therapy. The 3 parts of an anxiety problem: thoughts, emotions, and behaviors. The different strategies for approaching each of these 3 parts. The skill of how to cut off the worry process and redirect attention back to what you are actually doing in the present moment.  This is a critical but difficult “mental behavior” to learn, and we typically spend a lot of time in therapy working on this skill. [/vc_column_text][/vc_accordion_tab][vc_accordion_tab title="Step 3: Behavior Changes (typically included throughout the first few sessions)"][vc_column_text] Changing what you do in response to anxiety is what really leads to anxiety getting better in the long run. Based on the foundational concepts in the psychoeducation step, your therapist will review all of the behavior changes they see as being critical to your recovery and go over how to implement them.  This typically means eliminating behaviors designed to help avoid or resist anxiety in the short-term (because that’s what keeps anxiety going in the long term) and returning to normal activities even in spite of anxiety. [/vc_column_text][/vc_accordion_tab][vc_accordion_tab title="Step 4: Core Treatment Methods (typically 5-15 sessions)"][vc_column_text]This is the part that takes up the bulk of your therapy sessions.  Your therapist will decide which of these parts to prioritize and then they will go through them with you over the course of several sessions each. Once you have learned and practiced the basic skills and exercises, your therapist will continue to update and repeat these as needed. Skill Training and Practice: You will learn key skills that help with anxiety and go through various exercises to practice these skills in session with your therapist. Typically, the most important skill is to purposely leave worry questions unanswered while redirecting attention onto the present moment.  Mindfulness meditation is another key skill that can be very helpful and your therapist may teach you specific mindfulness exercises for anxiety and how to use them most effectively. Exposure Exercises: The core of Exposure Therapy is to practice confronting (rather than avoiding) the anxiety triggers to gradually train your brain through experience that they are not dangerous. What this looks like depends on the content of your fears. Sometimes it means confronting something physically like purposely inducing panic sensations for panic disorder or touching feared surfaces in contamination OCD. But it can also mean confronting anxiety-provoking thoughts, for which we often use a technique called “imaginal exposure”. This involves having us make a recording of the anxiety-provoking thoughts and then having you listen to it repeatedly without doing typical avoidance behaviors in order to train your brain that those thoughts are not dangerous. This causes the thoughts to lose their emotional impact. Exposure exercises often change as you progress through therapy and we figure out different nuances of your anxiety triggers, and as we move up to more and more difficult exercises in your “exposure hierarchy”. Homework: Between sessions, you will practice the skills and implement the behavior changes that you work on during sessions. You will also practice the exposure exercises.  Homework is actually the most important part of therapy: change does not actually happen during therapy sessions themselves, it really happens when you take what you learned in your therapy sessions and use it. [/vc_column_text][/vc_accordion_tab][vc_accordion_tab title="Step 5: Troubleshooting (typically 2-5 sessions)"][vc_column_text] Making big changes is hard and improvement is rarely a straight line upwards; it is more typically a gradual “2 steps forward, 1 step back” kind of process. The therapist is there to compassionately support you through this, keep you accountable and on target, and help troubleshoot barriers to success so that you ultimately reach your goals. [/vc_column_text][/vc_accordion_tab][vc_accordion_tab title="Step 6: Check-in sessions (typically 2-5 sessions)"][vc_column_text] Towards the end of therapy when we have taught you everything you need and things are improving, there is at some point less and less need for full 50 minute sessions every week and we typically start to titrate down the length and frequency of sessions. Most clients at some point start doing less frequent half sessions (25 minutes) for a little while until they have met their goals and maintained that success for a little while. [/vc_column_text][/vc_accordion_tab][vc_accordion_tab title="Step 7: Ending therapy"][vc_column_text] Cognitive-behavioral therapy is not meant to be a never-ending, lifelong process: the idea is that at some point (usually a relatively short period of time), we have given you everything you need: goals have been met, suffering has been minimized, and you no longer need us! The skills you learned in therapy become new, healthy habits that will serve you for the rest of your life. Our goal in our counseling is to help you start getting relief from anxiety as quickly as possible. 4. The type of therapy we do typically works relatively quickly. We know you want relief and it is our goal to show you how to make that happen as quickly as possible. On average, a full course of treatment at our practice typically takes about 10 to 20 sessions, which is consistent with what research studies say about how long treatment usually takes for anxiety and OCD; so if you are coming in once a week (which is typical), treatment usually lasts several months. However, you should begin to see some gradual improvements earlier in the process once you start consistently implementing the strategies we give you on your own outside of sessions. Bear in mind that some clients have quicker treatments than this and some clients have longer (and occasionally much longer) treatments than this depending on what you need, but 10-20 sessions is a good rough estimate. It is fine if you prefer to watch 1 or 2 of the exercises and give yourself a few days to practice those before watching and practicing the rest of the exercises. In the meantime, you can move on to the rest of the videos even if you haven't done all the exercises yet. Also note that the first exercise, the listening exercise, does not need to be practiced, it is more of a demonstration. I'd also like to note that clients often tell me they have heard of and tried mindfulness before and feel they "are terrible at it" or "can't do it".  Keep an open mind and give it a try...I address this in the video, but there is actually no way to be bad at mindfulness unless you are not even trying. The problem is that it is often taught just slightly off-target enough to make you feel this way. I believe I teach it a little bit differently than most and you will find it useful. Remember that it is a skill to practice that has cumulative benefits over time, not a relaxation exercise to calm you down and it takes repetitive, consistent practice at it to see its biggest benefits.
